Climate change is the biggest global health threat of the 21st century.

Effects of climate change on health will affect most populations in the next decades and put the lives and wellbeing of billions of people at increased risk. During this century, earth’s average surface temperature rises are likely to exceed the safe threshold of 2°C above preindustrial average temperature. Rises will be greater at higher latitudes, with medium-risk scenarios predicting 2–3°C rises by 2090 and 4–5°C rises in northern Canada, Greenland, and Siberia. In this report, we have outlined the major threats—both direct and indirect—to global health from climate change through changing patterns of disease, water and food insecurity, vulnerable shelter and human settlements, extreme climatic events, and population growth and migration. Although vector-borne diseases will expand their reach and death tolls, especially among elderly people, will increase because of heatwaves, the indirect effects of climate change on water, food security, and extreme climatic events are likely to have the biggest effect on global health.

A new advocacy and public health movement is needed urgently to bring together governments, international agencies, non-governmental organisations (NGOs), com-munities, and academics from all disciplines to adapt to the effects of climate change on health. Any adaptation should sit alongside the need for primary mitigation: reduction in greenhouse gas emissions, and the need to increase carbon biosequestration through reforestation and improved agricultural practices. The recognition by governments and electorates that climate change has enormous health implications should assist the advocacy and political change needed to tackle both mitigation and adaptation.

Management of the health effects of climate change will require inputs from all sectors of government and civil society, collaboration between many academic disciplines, and new ways of international cooperation that have hitherto eluded us. Involvement of local communities in monitoring, discussing, advocating, and assisting with the process of adaptation will be crucial. An integrated and multidisciplinary approach to reduce the adverse health effects of climate change requires at least three levels of action. First, policies must be adopted to reduce carbon emissions and to increase carbon biosequestration, and thereby slow down global warming and eventually stabilise temperatures. Second, action should be taken on the events linking climate change to disease. Third, appropriate public health systems should be put into place to deal with adverse outcomes.

The European Commission welcomes the renewed commitment to strengthen the links between Environment and Health set out in the Final Declaration of the Ministerial Conference held in Parma on 10-12 March 2010. The document sets out clearly how to implement the environment and health process across Europe, based inter alia on the Children’s Environment and Health Action Plan for Europe (CEHAPE).

The Commission supports the focus given to key environment and health challenges, such as the impact of climate change on health and the environment, socioeconomic and gender inequalities and the burden of non-communicable diseases linked to environmental conditions and disasters.

Despite the progress achieved so far, addressing major preventable determinants and diseases, such as obesity, respiratory and cardiovascular diseases, remains a major challenge. Renewed efforts are required to properly address the environmental causes of such determinants and diseases. The Parma Ministerial Declaration provides a strong impetus to the implementation of effective actions to further reduce the burdens that arise from such diseases.

The Commission is committed to working with governments, civil society and with international organisations, in particular the World Health Organization, to support as appropriate the achievement of the goals set out in the Parma Declaration.

In implementing its European Environment and Health Strategy3 through the EU Action Plan on Environment and Health4, the Commission will ensure that synergies between EU level actions and those arising from the Parma Conference are fully exploited.

Aspectos técnicos y sanitarios en la producción de agua de consumo humano

Transcurridos prácticamente cinco años desde la iniciación de los trabajos de composición de esta Guía, elaborada por el Ministerio de Sanidad y Política Social, con la participación del propio Ministerio, del CEDEX y expertos de las Asociaciones, AEAS, AEDYR, AQUAESPAÑA, se presenta para conocimiento general en las dos jornadas programadas. La Guía, fue presentada por la Autoridad Sanitaria en Las Palmas de Gran Canaria el pasado 15 de Enero, a los Directores de Salud Publica de las Comunidades Autónomas y técnicos en aguas de los servicios correspondientes.

Dada la importancia y trascendencia de la misma, organizamos estas jornadas en las que ponentes de la misma explicaran su contenido y objetivos, que podrán ser comentados abiertamente en el tiempo previsto para ello al concluir los tiempos de exposición. Las Jornadas, servirán para poner sobre la mesa este avance, que ha supuesto un esfuerzo de realización considerable y que ha tenido como uno de sus objetivos principales, el incorporar requisitos mínimos en el ámbito y practicas de la desalación en pos de la protección de la salud de los consumidores, aplicando nuevas y avanzadas tecnologías de tratamiento del agua.

Climate change endangers human health, affecting all sectors of society, both domestically and globally. The environmental consequences of climate change, both those already observed and those that are anticipated, such as sea-level rise, changes in precipitation resulting in flooding and drought, heat waves, more intense hurricanes and storms, and degraded air quality, will affect human health both directly and indirectly. Addressing the effects of climate change on human health is especially challenging because both the surrounding environment and the decisions that people make influence health. For example, increases in the frequency and severity of regional heat waves—likely outcomes of climate change—have the potential to harm a lot of people. Certain adverse health effects can probably be avoided if decisions made prior to the heat waves result in such things as identification of vulnerable populations such as children and the elderly and ensured access to preventive measures such as air conditioning. This is a simplified illustration; in real-life situations a host of other factors also come into play in determining vulnerability including biological susceptibility, socioeconomic status, cultural competence, and the built environment. In a world of myriad “what if” scenarios surrounding climate change, it becomes very complicated to create wise health policies for the future because of the uncertainty of predicting environmental change and human decisions. The need for sound science on which to base such policies becomes more critical than ever.

Recognizing the complexity of this issue, an ad hoc Interagency Working Group on Climate Change and Health (IWGCCH)2 assembled to develop a white paper on relevant federal research and science needs, including research on mitigation and adaptation strategies. Examples of mitigation and adaptation research needs are identified, but a comprehensive discussion of these issues is not included. These research and science needs broadly include basic and applied science, technological innovations and capacities, public health infrastructure, and communication and education. Consideration is also given to the potential structure of a federal climate change and health research agenda and the use of scientific research results for applications and decision making. The purpose of this paper is to identify research critical for understanding the impact of climate change on human health so that we can both mitigate and adapt to the environmental effects of climate change in the healthiest and most efficient ways. Although the group recognizes the global nature of climate change’s impacts on human health, the primary focus of this paper is on the situation in the United States.

This report is organized around 11 broad human health categories likely to be affected by climate change.3 Categories are arranged in alphabetical order, and no prioritization—for instance as to likelihood of occurrence, severity of effects, or depth of current knowledge—is implied. Each category is broken into sections that introduce the topic, explain its relationship to climate change, and identify the basic and applied research needs of that category, as well as crosscutting issues where relevant. Most investigations of climate change and health have relied on environmental and ecological effects to extrapolate potential human health impacts; the IWGCCH deliberately chose to emphasize the need for research on human health outcomes over environmental impacts for this reason: this approach highlights direct links between climate change and federal research priorities that are often disease- or outcome-specific, and a focus on human health outcomes enables a holistic approach to exploring climate change-related health impacts. We recognize that the health consequences identified in this document are not exhaustive, and that because so many climate change effects are prospective, some of the research needs enumerated may be speculative. As more information becomes available, new research needs may be identified and others rejected, but it is our intent that this report may serve as a baseline discussion from which agencies can proceed.

La presente convocatoria está dirigida a aquellas personas que tengan interés en ser parte del Comité Editorial de Gaceta Sanitaria, como editores/as asociados/as. Se
comprometerán a realizar las siguientes actividades: Revisar los manuscritos recibidos e identificar los revisores apropiados, solicitar artículos de personas expertas, promover y difundir la revista en congresos, asistir a las reuniones presenciales o telefónicas del Comité Editorial y velar por la mejora de la calidad científica de la revista, así como a las actividades propuestas por la dirección de la revista.

Como requisitos, las personas candidatas deben tener experiencia en investigación en los diferentes ámbitos de la salud pública y la administración sanitaria y conocer los principios de la revisión por expertos (peer review) por la que se rigen las revistas de ciencias de la salud. Esta experiencia se acreditará mediante

• la publicación de artículos científicos en revistas indizadas, preferentemente como primer autor/a o como co-autor/a, y
• la acreditación de haber colaborado en la revisión de manuscritos para Gaceta Sanitaria u otras revistas científicas

Como se establece en el mencionado reglamento, las nuevas directora y directora asociada, asesoradas por Esteve Fernández (director saliente de la revista) seleccionaran, entre las candidaturas recibidas de acuerdo a criterios de excelencia investigadora y experiencia editorial, las cuales propondrán a la junta directiva para su nombramiento. En la selección, se tendrá en cuenta además, la resultante distribución territorial, por sexo y por disciplinas.

Las solicitudes constarán de una carta explicativa de las motivaciones e interés por presentarse, junto a un breve currículo vitae donde consten los méritos que desean
hacerse valer. Las solicitudes deben dirigirse en formato electrónico a la Secretaría de SESPAS (sespas@suportserveis.com) antes del próximo 10 de julio de 2010.

En Barcelona, a 11 de Junio de 2010

La Fundación BBVA colabora por décimo año consecutivo con la Asociación de Economía de la Salud (AES) en la organización del taller Aplicaciones de Disease Mapping a la Economía de la Salud y a la Investigación en Servicios Sanitarios, que precederá a las XXX Jornadas de Economía de la Salud. El objetivo de este taller es ofrecer a los/as participantes una visión general de los métodos estadísticos más utilizados en el análisis geográfico de fenómenos en salud, mostrando la utilidad de estos en la evaluación de los sistemas sanitarios. Los modelos presentados se ilustrarán con aplicaciones en el análisis de las variaciones en la práctica médica, que son aquellas variaciones sistemáticas en las tasas de un procedimiento clínico particular entre áreas sanitarias. En concreto, se describirán experiencias de aplicaciones de la metodología en otros países, como por ejemplo la desarrollada en el Programa de Mejora en la Calidad y la Investigación de los Servicios de Salud Neonatal en Canadá, a través de las cuales se mostrará cómo estos modelos pueden ser útiles en la identificación de las necesidades de un sistema sanitario.

Los tres ponentes, los doctores Nicky Best, Ying Macnab y Peter Congdon, son reputados investigadores con importantes aportaciones en la materia y con experiencia en la aplicación de estos modelos en la investigación sanitaria. El taller es por tanto una excelente oportunidad para conocer la modelización estadística adecuada en el análisis de la variabilidad geográfica en el contexto de la investigación en servicios sanitarios.

Los servicios sanitarios (¿cúales, para quién, cómo?, pregunta el lema de las Jornadas) son el eje central de las XXX Jornadas de Economía de la Salud, que se celebrarán en ADEIT, Fundació Universitat Empresa de Valencia. Como en
ocasiones anteriores, las Jornadas están dirigidas a economistas de la salud y a un amplio conjunto de decisores y profesionales sanitarios con responsabilidades de gestión, interesados en aspectos de calidad y sistemas de información,
y a todos aquellos investigadores y profesionales que intentan orientar la toma de decisiones mediante la aplicación de metodologías sólidas y rigurosas.

El lema de las XXX Jornadas de Economía de la Salud, “Servicios sanitarios: ¿cuáles?, ¿para quién?, ¿cómo?”, coincide con la tercera pregunta del clásico de Fuchs, Who shall live? y expresa las preocupaciones inmediatas de nuestras sociedades por cómo mejorar el conjunto de decisiones clínicas, gestoras, ciudadanas y políticas que determina cómo cada sociedad establece sus prioridades.

Esta priorización se realiza en todas las sociedades del mundo, con denominaciones que admiten grados varios de eufemismo, de forma más o menos democrática, más o menos descentralizada, más o menos explícita, más o menos basada en el conocimiento, más o menos orientada al usuario, interiorizando en grado diverso el coste de oportunidad. De formas muy diversas, pero se realiza.

Todo el conocimiento universal y contrastable, proveniente de cualquier disciplina, que permita mejorar todas esas decisiones mencionadas (clínicas, gestoras, ciudadanas, políticas) será ciencia de la salud y también de la gestión y, sobre todo, podrá contribuir a la solvencia y justicia del Estado del Bienestar y a la solución de los múltiples desequilibrios que en estos momentos afectan a España. Las XXX Jornadas tienen como objetivo esencial incrementar ese conocimiento contrastable que contribuya a las soluciones que necesitamos. Y los contenidos de este programa ya anuncian que este objetivo se verá sobradamente cumplido.
